Recording Consent

Please fill out and submit at least 3 days before your first appointment.

A requirement for practitioners of The Richards Trauma Process Training (TRTP™) is to record their sessions with you. This is for both your benefit and theirs. You will be given a copy of the first session. The practitioner will listen to it for their ongoing training and professional development.

Recordings are treated confidentially and are never used for any other purpose. They will be listened to by their Supervisor. If you agree, please submit the form below.

I authorise Wendy Hayes to record our sessions for the purpose of enhancing our work together and for the recording to be used as stated above.
